Cardiac glycosides block Na K ATPase, increasing sodium calcium exchange thus increasing intracellular Ca, with the following effects:

  • positive inotropism
  • disturbance of rhythm by:
    • blocking the AV conduction
    • increasing ectopic pacemaker activity
  • negative chronotropy, mediated via increased vagal activity
