The main treatment of cryptogenic organizing pneumonia is with corticosteroids such as prednisolone.

Response is variable but in many cases improvement occurs rapidly.

In the majority of idiopathic cases, where there appears to be no associated illness complete resolution occurs in 65-85%. This treatment however appears less effective in patients with dermatomyositis, immunosuppression or interstitial opacities at lung bases.

Respiratory failure leading to death may occur in 5% of patients.
