  • described by Gordon Holmes in 1904 as a 3-4 Hz coarse tremor of the arms present at rest and exacerbated by action and posture
  • tremor tends to involve the proximal musculature
  • is secondary to vascular or traumatic lesions in the midbrain and in multiple sclerosis
  • has variable response to medications including levetiracetam, levodopa and trihexyphenidyl
