Treatment of atrial flutter

The main goal of treatment is to cardiovert to sinus rhythm. Prioritise identification and treatment of any underlying cause as in about 60% of patients, atrial flutter occurs as part of an acute medical illness, and will resolve when this is treated (1)

There are three methods of cardioversion:

  • DC shock
  • pharmacological
  • overdrive pacing

If sinus rhythm is not achieved then the rate of ventricular response should be controlled.

NICE has issued guidance on the management of atrial fibrillation (AF), including people with atrial flutter. (2)
