Inotropes should be considered in a patient with persistent left ventricular failure and hypotension despite an adequate filling pressure (wedge pressure 14-16 mmHg):

  • dobutamine infusion:
    • inotrope
    • 2.5-10 mcg/kg/minute, titrate against response

  • dopamine:
    • selective renal vasodilator
    • initial dose 2mcg/kg/minute
