The inferior surface of the first rib is relatively smooth compared to superior surface:

  • posteriorly it is crossed obliquely by the first intercostal nerves and vessels
  • medially, the cervical pleura is attached and passes anteriorly in a broad band
  • at the point of greatest curvature, fibres of serratus anterior can arise
  • anterior to the tubercle at the most lateral edge, the intercostal muscles arise
  • anteriorly on the medial edge, the suprapleural membrane begins to arise
