Abscess (perinephric)

This may occur as a complication of pyelonephritis, particularly if there is a large 'staghorn' calculus present, when the pus discharges through the renal capsule into the perinephric fat. Also may develop as a result of haematogenous infection of a traumatic perinephric haematoma.

Presentation:

  • systemic symptoms of infection
  • local low-grade symptoms
  • pyuria

Investigations:

  • ultrasound or radiology may reveal a non-functioning renal mass which contains a fluid level

Treatment:

  • drainage of the abscess
  • nephrectomy may be required
