The severe photosensitivity, red fluorescent urine and erythrodontia are usually diagnostic of erythropoietic porphyria (EP). A differential diagnosis of EP includes:

  • xeroderma pigmentosum
  • epidermolysis bullosa - but haemolytic anaemia is rare in this condition, and it is common in areas of repeated trauma rather than in light-exposed areas
  • pemphigoid
  • erythropoietic protophorphyria
