Clinical features
Intramedullary lesions produce segmental features only. These develop gradually and include:
- segmental pain
- bilateral loss of pain and temperature sensation - distribution is governed by segments involved due to interruption of the decussating fibres to the lateral spinothalamic tract
- lower motor neurone weakness - site determines muscle groups affected
- upper motor neurone weakness below the level of the lesion - from corticospinal tract involvement
- loss of proprioception - a late feature
