Intramedullary lesions produce segmental features only. These develop gradually and include:

  • segmental pain
  • bilateral loss of pain and temperature sensation - distribution is governed by segments involved due to interruption of the decussating fibres to the lateral spinothalamic tract
  • lower motor neurone weakness - site determines muscle groups affected
  • upper motor neurone weakness below the level of the lesion - from corticospinal tract involvement
  • loss of proprioception - a late feature
