Otosclerosis is inherited in an autosomal dominant pattern with variable penetrance but in about 35-50% of cases, there is no family history of deafness.

Frequently, it is precipitated or promoted by pregnancy, and may also complicate conditions such as Pagets' disease and osteogenesis imperfecta - Van der Hoeve's syndrome.
