Lord's procedure involves manual dilatation of the anal sphincter under general anaesthetic. It is a adjunctive procedure in the early stages of piles. It results in a lowering of intra-anal pressure during defaecation, thus making the passage of stool easier. It is thought to reduce the discomfort of defaecation. With a reduction in straining, there is less vascular congestion of the anal cushions.
Note that this procedure, if performed in an uncontrolled manner, may cause permanent faecal incontinence, especially in elderly patients.
