Dosage of methotrexate

- start at 5mg or 10 mg oral or IM WEEKLY
- folic acid 5 mg oral weekly, 3 days after methotrexate reduces adverse reactions
- if inadequate response dose may be increased upto 20 mg weekly
- total duration of treatment as long as clinically indicated
