Patients with structural heart disease or abnormal ECG

Syncope in patients who have known structural heart disease or abnormal ECG is most often caused by arrhythmias.

The following investigations should be considered:

  • echocardiography in most patients
  • exercise testing is particularly informative if syncope occurs on exertion
  • 24 hour ECG monitoring

A referral to a cardiologist should also be considered.
