The various pathological processes in multiple sclerosis can be imaged using various MRI protocols:

  • inflammation:
    • gadolinium DTPA enhancing lesions on T1-weighted images are of recent origin and acutely inflamed
  • demyelination:
    • magnetisation transfer ratio
  • astrocytosis:
    • can be seen as areas of increased water content on T2-weighted images
  • axonal damage:
    • reduction in diffusion tensor imaging anisotrophy
    • N-acetyl-aspartate spectroscopy
    • focal atrophy
    • T1-weighted black holes
