Common causes
These include:
- developmental feature: puberty
- familial feature
- insufficient sleep
- malnutrition
- psychological factors e.g. school phobia, depression
- hypoglycaemia
- anaemia
- infection - low grade, such as pyelonephritis, tuberculosis, persistent haemolytic streptococcal throat infection, or early rheumatic fever
- apical tooth infection
- glandular fever
- chronic or subclinical hepatitis
