This is a deformity seen in rheumatoid arthritis where there is hyperextension at the proximal interphalangeal joint and flexion at the distal interphalangeal joint. It is caused by softening of the PIP capsule, and it's appearance has been likened, by imaginative doctors, to the curve of a swan's neck.
