TUR syndrome

The TUR syndrome is an uncommon complication of transurethral surgery attributed to the absorption of irrigating fluid during the operation and in the immediate postoperative period.

Features include:

  • confusion
  • hypotension
  • bradycardia
  • nausea and vomiting
  • collapse

Investigations reveal:

  • hyponatraemia
  • hypovolaemia
  • sometimes, hyperammonaemia due to metabolism of absorbed glycine

Management:

  • discontinue irrigation
  • diuretics
  • normal or twice normal saline
  • seek senior advice
