When using an ACE inhibitor, a loop diuretic should be retained but amiloride should not. This is because the combination of a potassium sparing diuretic and amiloride may cause hyperkalaemia and therefore potential heart block.
There is evidence, via the ATLAS study, that a high dose ACE inhibitor may be a more effective treatment intervention than a low dose ACE inhibitor.
