  • ischaemia
  • thick chest wall or emphysema
  • pericardial effusion
  • cardiomyopathy or myocarditis
  • constrictive pericarditis
  • hypothyroidism
  • hypoadrenalism
  • hypokalaemia
  • hypocalcaemia
