Diagnosis is by isolation of Clostridium botulinum from the patient's faeces, or suspect food, or by detection of toxin in the patient's serum or faeces.

Repetitive nerve stimulation shows a decrement in the evoked muscle action potential at slow rates of stimulation, but an increase in amplitude during stimulation at faster frequencies - a typical presynaptic defect.
