The patient is positioned lying on his back. A slight head-down tilt may facilitate access to the inguinal region. The area is prepared with an antiseptic solution and then draped to expose the length of the groin crease to the penis and scrotum.

If the patient is relatively fit and has good social support, then they may be booked as a day case. If this is the case, the patient must know before the operation that they cannot drive home.
