Pes cavus can be managed conservatively or by surgery.

Conservative treatment:

  • often no treatment is required
  • special shoes may be needed for adults with a fixed deformity

Surgery:

  • if the deformity is still mobile then one should consider a tendon rebalancing operation while the condition is still correctable e.g. transplanting the flexor into the extensor tendons
  • if there is a fixed deformity then arthrodesing the toe joints is the only option
  • any complex operations on the arch itself should be delayed until the patient is 16 years old
