In the Old World, cutaneous leishmaniasis is spread by female sandflys of the genus Phlebotomus; in the New World the genus is Lutzomyia.

Urban infections, especially in India, are not zoonoses - humans are the only reservoir of infection. In rural populations other mammals may be involved in the cycle of infection e.g. dogs, gerbils, rats.
