The diagnosis is usually based upon the clinical presentation, including the timecourse of symptoms following exposure to a putative allergen.

The presence of specific serum precipitins are supporting evidence for the diagnosis.

In difficult cases, for example where the putative allergen is novel, further tests, include lung biopsy, may be required.
