Acute pyelonephritis is characterised by an acute onset of pain in the loin which radiates to the iliac fossa and suprapubic area.

Other features are:

  • general systemic disturbance - sudden onset of fever, rigors, vomiting
  • urinary frequency
  • painful micturation
  • cloudy, offensive urine
  • tenderness and guarding over the kidney

In children the systemic features are predominant.
