  • bacterial infection
  • surgery
  • burns
  • trauma
  • haemorrhage
  • infarction
  • inflammation
  • polymyalgia
  • myeloproliferative disorders
  • leukaemias
  • polyarteritis nodosa
  • drugs e.g. steroids
