A diagnosis of dermatomyositis requires that five features are present:

  • proximal, symmetric muscle weakness
  • muscle biopsy evidence of myositis
  • elevation of serum muscle enzymes
  • characteristic electromyographic abnormalities
  • compatible skin involvement

Polymyositis requires only four features, there being no skin involvement.
