Biliary colic may be strongly suggested by the history but the clinical impression should always be verified by appropriate investigations such as ultrasound.

Other diseases with similar pain include:

  • duodenal ulcer
  • gastro-oesophageal reflux
  • pancreatitis
  • myocardial infarction
  • right sided testicular pain in the T6 - T10 dermatomes

Irritable bowel syndrome may be mistaken for gallbladder discomfort.
