Classically, abdominal radiography reveals a triad of features:

  • gas in the biliary tree:
    • 40% of cases

  • a gallstone in the intestine:
    • often at the ileo-caecal junction

  • small bowel obstruction

When the clinical picture is unclear, an upper gastrointestinal series will reveal the cholecystoenteric fistula and verify the intestinal obstruction.
