ATLS: fluid replacement regimen in established shock

Authoring team

Immediate fluid replacement consists of:

  • 1 litre colloid and 1 litre crystalloid stat
  • ask for blood - O neg or group specific
  • send samples for typing / cross-match

IF CONDITION CONTINUES TO IF CONDITION CONTINUES TO IMPROVE: DETERIORATE:

  • Maintenance crystalloid þ Give blood
  • Monitor patient þ Give 1 litre crystalloid



  • Await cross-matched blood þ Give blood
  • Identify bleeding source
  • Transfer to theatre

ALL FLUIDS, ESPECIALLY BLOOD, SHOULD BE WARMED
