Ophthalmologic involvement is common in Graves' disease, and it accounts for the majority of dysthyroid eye disease. Typically, it develops within 12 months of the onset of hyperthyroidism but may precede it or develop concurrently with it.

Very rarely, dysthyroid eye disease may occur in the presence of autoimmune primary hypothyroidism.
