Rarely, shoulder pain is due to infraspinatus tendinitis. Tenderness is maximal around the point of insertion, behind and below the tip of the greater tuberosity, with the pain provoked by external rotation.

The condition is managed conservatively with injection of corticosteroid directed to the point of maximal tenderness.
