Surgery for carcinoma of the middle third of the oesophagus entails a partial oesophago-gastrectomy. The site may be approached either through a right thoracotomy or thoraco-abdominally via the beds of the fifth and eighth ribs.

The diseased length of oesophagus and approximately one fifth of the stomach is removed. It is anastomosed to the oesophageal remnant at the level of aortic arch.
