Posterior subcapsular cataract

This is the most common type of ageing change. An opacity is formed in the cortical region of the lens adjacent to the posterior lens capsule and appears as a central black spot against the red reflex.

The visual axis is obscured early and reduced acuity may be particularly noticeable when the pupil constricts, for example in reading or bright sunlight. These patients have :

  • visual difficulty in good lighting but less trouble when the light levels are low (due to pupil dilation)
  • difficulty in day time driving and reading (1)
