The clinical features of neuromyelitis optica are:

  • rapid, bilateral and occasionally, complete, visual loss

  • spinal cord symptoms follow hours, days or weeks later after visual signs:
    • back and girdle pain
    • paraesthesia and weakness in lower limbs
    • extensor plantars
    • urinary retention

There may be a preceding upper respiratory infection.
