If antibiotic therapy is started promptly then the progression from acute to chronic osteomyelitis may be prevented.

The bone around the site of infection is initially osteoporotic but eventually becomes sclerotic and thickened.

There may be remodelling of the bone with complete resolution. Commonly the bone is left permanently deformed.
