    • autosomal recessive (Lange-Nielsen syndrome)
  • drugs:
    • disopyramide
    • procainamide
    • amiodarone
    • sotalol
    • phenothiazine
    • ketanserin
    • non-sedative antihistamines e.g. terfenadine
  • electolyte disturbance e.g. hypokalaemia
