Possible causes of acute mediastinitis include:

  • perforation of the oesophagus
  • perforation of the trachea
  • direct extension of infection from lung, pleura, or elsewhere above or below the diaphragm
  • infection at time of opening of the chest wall, for example during cardiac surgery
