There is good evidence that patients with symptomatic carotid artery stenosis >70% benefit from carotid endarterectomy.

In calculating whether surgery is indicated for a particular patient one should consider:

  • the risk of further strokes:
    • TIA's suggest high risk
    • large completed strokes reduce the utility of ipsilateral surgery

  • the operative risk of complications:
    • operator dependent
    • avoid surgery soon after a stroke or myocardial infarct
