  • from calcium antagonist - discontinue calcium antagonist and initiate angiotensin-II antagonist

  • beta blocker - do not withdraw the beta-blocker immediately, but titrate in accordance with the manufacturer's instructions and initiate therapy with angiotensin-II antagonist

  • ACE inhibitor - if the patient is already stabilised on the ACEI, discontinue the ACEI, and initiate therapy with losartan

  • diuretic - discontinue the diuretic 2-3 days before starting angiotensin-II antagonist
