Surgery is indicated in an asymptomatic patient if they have a low diastolic blood pressure, dilated left ventricle and evidence of left ventricular hypertrophy.

Operative procedures include:

  • reposition of cusp - this procedure is undertaken when the regurgitation is due to a prolapsed cusp through ventricular septal defect, or to the dissection of the ascending aorta.
  • aortic valve replacement - for patients other than above- some suggest the use of pulmonary allografts as the optimal substitute for the diseased aortic valve in children and young adults.
