The clinical features of neuralgic amyotrophy are:

  • the patient usually complains of the sudden onset of an intense pain in the shoulder and arm followed in a few days by muscular weakness and wasting
  • weakness is usually proximal but may affect the entire arm
  • occasionally, both arms are affected simultaneously
  • sensory features are less apparent and occur in 50% of cases - sensation tends to be lost over the outer aspect of the shoulder, usually follows the pain, and may occur without subsequent wasting
  • there is usually reflex loss
