ECG changes in right bundle branch block (RBBB)

Right bundle branch block is best seen in V1 where there is an RSR (M-shaped) pattern.

  • QRS duration greater than 120ms RSR
  • generally dominant R in V1
  • inverted T waves V1-3 or V4
  • wide and deep S in V6
