ECT is only indicated in the presence of depressive symptoms since it appears to have little effect anti-obsessional effects.

Psychosurgical centres also practice bimedial leucotomy. Although short-term benefits are good, with reductions in anxiety and obsessional symptoms, the long term results are not known. Psychosurgery should be seen as a last resort in severe OCD which fails to respond to drugs and psychotherapy.
