Aetiology
The common cause is viral infection with Coxsackie virus.

- thiamine deficiency
- thyroid disease
- amyloidosis
- connective tissue disease:
- SLE

- infiltrative sarcoidosis and leukaemia
- neuromuscular:
- Friedreich's ataxia
- Duchenne muscular dystrophy
- drugs:
- aresenic
- alcohol
- phenothiazines
- radiation
