Chronic conditions

Authoring team

Regular supervision and periodic reassessment of patients with conditions such as hypertension, diabetes and epilepsy allows:

  • therapy to be reviewed
  • compliance to be checked
  • complications to be ancicipated or identified at an early and remediable stage
  • continuity of care, preferable in general practice to seeing a different junior hospital doctor at each visit to the outpatients' department
  • encouraging a lifestyle likely to produce the best outcome
  • continuing psychological support to help the patient cope with any permanent disability
