Internal mammary lymphatic metastasis tends to come from the medial half and the periareolar regions of the breast. Spread to the internal mammary chain occurs with a lesser frequency than axillary nodal spread but it has a similarly poor prognosis.
Surgery or radiotherapy to involved nodes does not seem to improve survival.
