New patient registration

Authoring team

A new patient registration consultation must be offered to all patients over 5 years old. The consultation is to establish relevant past medical and family history including:

  • medical factors - illnesses, immunizaitions, allergies, hereditary factors, screening tests, current health
  • social factors - employment, housing, family circumstances
  • lifestyle factors - diet and exercise, smoking, alcohol and drug abuse

The examination must include height, weight and blood pressure.
