The local spread of breast carcinoma occurs by three main mechanisms:

  • infiltration of surrounding tissue - direct extension:
    • main route of local spread
    • may extend superficially to skin or deep to pectoralis fascia
  • spread along ducts
  • local lymphatic dissemination
  • local vascular dissemination
